Non-Medical Case Manager - Center for Development and Disability

The Center for Development and Disability (CDD) is seeking a Non-Medical Case Manager to join our team! This opportunity is a Full Time day shift opening located in Albuquerque, New Mexico.
The position of Non-Medical Case Manager will work within the multidisciplinary team, and will support clinicians to provide comprehensive diagnostic evaluations and family-centered care.
This position will support families referred for evaluation, complete new client intakes, insure that documentation is requested and received, assess client and family needs.
Non-medical Case Managers provide information regarding services and supports which may be available to the client and family while they are waiting for an evaluation.
The position will perform clinical administrative activities to support program quality and efficiency.
The position will work closely with the clinical team to insure that families and their children are able to obtain the evaluation without regard to language, geographic distance from the clinic, or other barriers.
Minimum $38,521 - Midpoint $48,172 Salary is determined based on years of total relevant experience.
Salary is based on 1.
0 FTE (full time equivalent) or 40 hours per week.
Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

Fluent in Spanish Experience working with confidential patient information, electronic medical record, and clinical documentation.
Knowledge of New Mexico developmental disabilities, mental health and behavioral health systems.
Summary Under general supervision, as part of a multi-disciplinary team, provides services relating to improving retention in care for specialized subsets of patients.
Ensures eligibility for a variety of medical and social services and assists patients in completing appropriate paperwork.
Non-medical case managers may provide assistance in accessing social, community, legal, financial and other needed services.
May perform a range of administrative functions required to ensure efficient program operation.
Minimum Job Requirements of a Non-Medical Case Manager:
High School diploma or GED and 2 years of directly related experience.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
Verification of education and licensure will be required if selected for hire.
Duties and Responsibilities of a Non-Medical Case Manager:
Coordinates patient documentation to maintain eligibility for a variety of core medical and social support services; Conducts patient interviews required to complete intake applications and all required forms for core medical and social support services; Works closely with medical case managers to reduce patient barriers to obtaining needed medical care and /or social services; Assist with the identification of community resources that build support for medically and socially complex patients; Maintain current information on all regularly accessed community resources; Perform a range of administrative functions required to ensure efficient program operation.
Maintain professional growth and development through completion of required departmental and New Mexico Department of Health competencies.
Perform miscellaneous job-related duties as assigned.

About UNM Comprehensive CDD:
This position will be located at the Center for Development and Disability (CDD).
Established in 1990, CDD is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service.
These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.
The Autism Programs at the CDD is a large, multi-faceted program with multiple funding streams and projects that support individuals with autism spectrum disorder and their families to live the life of their choosing.
This is accomplished through clinical evaluation/diagnosis, extensive community training, parent-mediated training, consultations, and technical assistance.
Currently our Autism Spectrum Evaluation Clinic evaluates approximately 300 clients per year with suspicion of autism spectrum disorder.
The latest prevalence rates for ASD are 1 in 36 children; this has steadily increased over the past several years and as a result the demand for our evaluations have increased.
We currently have over 1200 clients on our waiting list.
