Summary:
Facilitates Hospice and Palliative Care referrals from all sources and obtains reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program. The position assumes dual accountability for Palliative Care and Hospice referrals.

Associates Degree or Diploma in Nursing minimum
New Mexico RN License required.
Minimum one year of acute care experience required and hospice experience strongly preferred. Basic typing competency.
Current driver's license and auto insurance.
